超高齢社会における問題点として骨粗鬆症と骨折を取り上げ、概説した。薬剤の開発により欧米の先進国では、骨折が減少傾向であるが、本邦ではいまだに増加し続けている。この傾向は2040年に31万8千人でピークに達する。さらに骨折に関連する社会保障費についても概説した。後期高齢者の入院費を骨折と脳卒中が毎年1位、2位を占めているが、脳卒中に関しては生活習慣と密接な関連が明らかにされ、高血圧症、糖尿病、高脂血症、飲酒、喫煙など予防すべき危険因子が指摘された。現在では要介護となる疾患のうち脳卒中は減少傾向にある。一方、高齢者の骨折は取り残されたままである。対策が必要である。 |

Osteoporosis and fractures are well-known problems in aging societies. The numbers of osteoporotic hip fractures are decreasing in the developed countries in Europe and the United States due to drug development; however, in Japan the numbers are still increasing. If this trend continues, the number of the osteoporotic hip fractures will peak at 318,000 in 2040.The social security costs associated with fractures must also be addressed. Costs associated with fractures and strokes occupy the 1st and 2nd ranks respectively of hospitalization costs for the elderly. However stroke-related costs have been decreasing due to prevention of lifestyle-related diseases closely related to stroke, such as hypertension, diabetes, hyperlipidemia, drinking and smoking. Still, the problems of fractures among elderly people remain unresolved, and further measures are necessary. |
